2 Percent of smokers in each category who received physician counseling to quit smoking during the 12 months prior to the survey.
3 Persons without a physician visit were assumed not to have been counseled by a physician to stop smoking.
4 Includes all smokers who had a physician visit during the 12 months prior to the survey.
5 Includes all persons who smoked during the 12 months prior to the survey, including recent quitters.
6 Insured persons are those with private insurance, military insurance, Medicaid or other public insurance, Medicare, or any combination of these at the time of the survey. All others are considered to be uninsured.
7 Persons having a usual source of care are those indicating that they have a place that they usually go to when they are sick or need advice about their health. Persons having a usual provider are those indicating that they usually see the same provider during each health care visit.

9 Received physician counseling to quit smoking during the 12 months prior to the survey.
